Insurance Accepted and Payment Plans

We are currently accepting Anthem Blue Cross Blue Shield, United Healthcare, Cigna, HealthLink and Group Health Plan insurances as well as Medicare. If you have a different insurance we would be happy to contact them to determine your chiropractic benefits.

Individuals in need of our services who lack insurance and who are worried about the high costs charged by many other clinics need not worry. We have highly affordable rates and flexible payment plans allowing access to high-quality care for you and your family.

All of our patient forms require you to have Adobe Reader installed. If you do not have Adobe Reader, please install it by clicking on the link below.

Please review the Welcome Letter. You have an option of filling out our patient intake forms online or you may print out each form and bring them in with you during your next visit to the clinic. Please sign the Consent form, Patient Information form, and also the appropriate form for the region you are currently experiencing issues with.

If you are experiencing knee pain, please fill out the Knee Patello Femoral form.
